2023 Girls Youth Scholarship Form

This Scholarship application must be filled out by the player's guardian. We will be looking at these applications and granting scholarships to those players that are in need. If you have any questions, please email info@imlaxutah.org.

All applications must be submitted before Wednesday, February 1st 2023.

You will recieve notice about your scholarship the week of February 6th at the latest.
